RVSP and PH probability
Systolic PA pressure from the TR jet and right atrial pressure, plus the ESC/ERS 2022 echocardiographic probability of pulmonary hypertension.
ASE 2010 right heart guidelines and ESC/ERS 2022 PH guidelines
How it's calculated
Pressures
sPAP (RVSP) = 4 × TR Vmax² + RAP Mean PAP = 4 × PR early velocity² + RAP PA diastolic pressure = 4 × PR end-diastolic velocity² + RAP PVR (Wood units) ≈ 10 × TR Vmax ÷ RVOT VTI + 0.16RAP from the IVC
- 21 mm or less, collapsing more than 50%: 3 mmHg.
- Over 21 mm, collapsing less than 50%: 15 mmHg.
- Any other combination: 8 mmHg.
Echo probability of PH (ESC/ERS 2022)
- TR velocity 2.8 m/s or less, or not measurable: low probability without other signs, intermediate with them.
- TR velocity 2.9 to 3.4 m/s: intermediate without other signs, high with them.
- TR velocity above 3.4 m/s: high probability.
sPAP equals RVSP only without RVOT obstruction or pulmonary stenosis. The PVR estimate is less reliable above 8 Wood units. Rudski LG, et al. J Am Soc Echocardiogr 2010;23:685–713. Humbert M, et al. Eur Heart J 2022;43:3618–3731. Abbas AE, et al. J Am Coll Cardiol 2003;41:1021–1027.
